In older adults, urinary tract infections often skip the classic symptoms entirely and show up as sudden confusion, agitation, hallucinations, dizziness, or falls — a state doctors call delirium. Families mistake it for a stroke, or for dementia arriving overnight. In someone who already has dementia, it looks like sudden, severe worsening. The key word is sudden: dementia changes over months and years — delirium changes in hours or days. What to do: any abrupt mental change means a medical evaluation and a urine test that same day. Treated with the right antibiotics, the confusion typically clears — though in elders it can take days or weeks to fully lift.
