Get legal authority in place — power of attorney and a healthcare agent — so hospitals will actually talk to you. Centralize everything: medication list, providers, insurance, and documents in one shared place the whole family can reach. Make visits count — check the refrigerator, the mail pile, the car, the hygiene, not just the pleasant lunch. And know the local resources before the crisis: which hospital, which agency, who answers at two in the morning.

The distance isn’t the real problem — the lack of eyes and authority is. Professional home care becomes your presence on the ground, with visit reports that replace guessing.
