Watch for the pattern, not the single event. A fall — or new bruises they explain away. Pills still sitting in the organizer. Weight loss, an empty refrigerator, spoiled food. The same story told twice in one visit. Missed appointments, unopened mail, unpaid bills. A once-tidy home starting to slip. Bathing and grooming less. Withdrawing from church, friends, and phone calls. Any one of these can be a bad week. Two or more, sustained over time, is a trajectory — and trajectories do not reverse themselves on their own.

Families wait for the crisis — the fall, the fire, the hospitalization — because starting the conversation feels like betrayal. It isn’t. Acting on the whisper is how you avoid deciding everything inside a forty-eight-hour discharge window.
