A geriatric care manager — today often called an Aging Life Care Professional — is typically a nurse or social worker whom families hire privately to manage a senior’s whole situation: assessing needs, building the plan, vetting and coordinating providers, attending doctor visits, monitoring regularly, and reporting back to out-of-town children. When they earn their fee: complex medical situations, long-distance caregiving, sibling conflict that needs a neutral expert voice, or families with more income than time. They charge hourly, private pay — assessments cost more, ongoing coordination less. For many families, a quality home care agency’s built-in care coordination covers much of this need; for truly tangled situations, a care manager can be the difference between chaos and a plan.
