There’s one person in the hospital whose entire job is planning what happens to your mother next. Most families never learn their name.
The discharge planner — usually a social worker or nurse case manager — coordinates where the patient goes after the hospital: home with support, home health, rehab, or a facility. They arrange the referrals, the equipment, and the transportation, working inside insurance rules and tight timelines. How to work with them well: introduce yourself on day one and ask to be included in planning. Be brutally honest about the home situation — who is actually there at two in the morning? Ask what every option costs and what insurance covers. If the plan isn’t safe, push back respectfully with the exact words: ‘I don’t believe this is a safe discharge.’ Those words carry weight. And get every referral, appointment, and instruction in writing.
The discharge planner controls the map — but you know the terrain. Families who engage early get better plans than families who meet the planner an hour before the wheelchair reaches the curb.
